Pretest Survey Worksheet: Instructions
Step 1: Unique Identifier. Create a unique method for your participant to identify
themselves to link their pre-test and post-test responses without revealing their
identity to you. The identifier should be something that the participant can readily
recall, but not identifiable to you.
(a) An example would be: First 3 letters of your mother’s first name, the
first 3 numbers of your driver’s license, and the last three letters of the street
you grew up on.
(b) An example of how this would look is: jes123dge

Step 2: Survey Questions. Write 5 questions for your peers. This is to help you gain
practice for your DNP project; however, your questions should NOT be specific to
your DNP project. You may find it helpful if you pick a “theme” to your survey,
but a cohesive theme is not required (ie, reading, baking, etc.). You will need to
fulfill the following criteria:
(a) The five questions should have ONE (1) each of the following levels of
measurement:

One nominal/categorical question.
One ordinal question.
One interval question.
One ratio question.
One qualitative question.
(b) At least ONE demographic question.
(c) At least ONE question that is subjective and could (or will change, possibly
arbitarily) based on the day (such as mood, weather temperature, tiredness,
clothing that day). Remember that the level of measurement chosen here will
impact your analyses later! Please indicate which question you intend to be
subjective by including – (subjective) – next to that question. Your
qualitative question should not also be your subjective question, as you
will not be able to perform any tests of group differences (ie, quantitative
analyses) on qualitative data.
(d) NO specialty specific questions (i.e., don’t ask specific NICU questions –
all classmates are from different specialty backgrounds and would not be able
to answer).
(e) NO political questions
Step 3: Answer Choices. Include your proposed answer choices for each question.
Remember that your answer choices are dictated by the level of data you want to
collect. For interval/ratio level questions, you may allow for a free-text numerical
response here. For qualitative questions, you should state open-ended text response.
Step 4: Planned Analyses.
Identify 2 planned descriptive data analyses for each of your 4
quantitative questions. Identify your planned analyses (only 1 is required) for your
qualitative question.
(a) Be sure to include TWO (2) planned analyses for each quantitative question
so that you can receive sufficient faculty feedback before proceeding with
your descriptive data analysis report.
(b) Analyses should be appropriate to the level of data you have collected.
(c) Descriptive analyses for quantitative data include:
• Measures of central tendency
o If you include measures of central tendency, please indicate
the specific tests you intend to perform (ie, mean, median,
etc.)
• Crosstabulations
o If you plan to perform crosstabs, please make sure to indicate
which variables are included in the crosstabulations.
• Frequency counts
