Using the concepts related to your change project and field experience/practicum, create and present a poster that describes key components of your capstone project. The recommended format for the poster is a single PowerPoint slide. A sample poster is included in Canvas to help guide the layout and general design. You may design the poster using your creativity as long as the primary components are included. It is recommended that you use Microsoft PowerPoint to create the poster. However, you may use any program or software of your choice to create the poster as long as the finished product can be submitted to Canvas and accessed/viewed by faculty and peers (without the need to install any additional plug-ins or software). Other software options include, but are not limited to, Microsoft Word or Keynote.
Components of the Poster:
In your poster, include the following nine (9) components:
Project title
Name and credentials: Include both academic and professional credentials (e.g., John Smith, ASN, RN)
Population/Setting
Who was your target population? Remember: For this project, patients/clients are not included in the target population.
The target population consists of the individual(s) who were directly impacted by the change component.
Who were the key stakeholders? For this project, key stakeholders almost always include patients/clients.
Key stakeholders often include members of the interdisciplinary team and even the entire healthcare organization.
Using general terms, describe the project setting.
Do not state the name of the specific healthcare organization or setting.
Maintain confidentiality and anonymity. Change component
What problem or issue did you aim to change or seek to improve?
What is the background of the problem or issue you identified? How did you identify or recognize the problem/issue? What evidence supports the existence of the problem/issue? For example, what were your personal observations related to the problem/issue? Based on your literature review, summarize one or two current key findings related to your problem/issue.
How was delivery of the change handled?
How did your preceptor guide and support the project development and implementation?
Desired short-term outcome: Discuss the desired short-term project outcome(s). That is, what did you aim to accomplish during Capstone Course?
Initial Assessment of Change
How did you evaluate the effects of your change component?
Did you achieve your desired short-term outcome? How does your evaluation criteria support your assessment?
What is the initial impact of your change component?
Desired long-term outcome
What is/are the long-term desired outcome(s) of your project? Explain the link between the short-term and long-term outcomes. That is, how will achievement of the short-term outcome promote achievement of the desired long-term outcome?
Conclusions & Recommendations
What conclusions did you draw from this project?
What challenges or barriers limited the successful implementation of your project? Should you replicate this project? Is so, what would you do differently? If not, why not? What are your future recommendations? References
