Telehealth visit was completed today with my client. Client was 35yr old, alert and oriented x4, verbalized stability, and denied suicidal ideation. Verbalized no skin bruise, cut, no discoloration, and no pain. Last medical appointment was July 2023. Client stated he lives in a group home with other resident. Verbalized intact skin, no bruise, cut, and no discoloration. Client was stable and no assistive device such as cane, walker and no wheelchair. He collects money every Friday’s and goes for grocery shopping. Client verbalized diagnosis of diabetes, hypertension, and hyperlipidemia. Client received nursing teaching on cause of diabetes, client education. Kindly follow the rubric in the attachment below.
