The purpose of the Capstone is to prepare an evidence-based improvement plan to meet the assessed health needs of a selected population. The student will identify the assessed health needs and risks of a selected population and develop a plan to meet a selected need. It is expected that the student will draw upon basic knowledge of human and cultural diversity to engage in therapeutic relationships with the targeted population. Using content learned in this course, the student will prepare a comprehensive plan to implement a preventative health project directed at the target population.
Instructions
Below you will find the required elements of this assignment. All submissions are expected to be well written, with proper grammar and spelling. Include a cover page, introduction, body, and summary/conclusion. Remember, this is a scholarly paper and must include citations of evidence-based literature. A minimum of 5 references should be used. The length of the paper should be between 6-8 pages excluding the title page and reference page. If your paper is any longer, please contact your instructor to help you to narrow down the scope.
Identify the selected target population. (1 page)
Introduction and thesis.
Why this population? Support your decision with data from the community indicating the need for the intervention.
What ethical considerations will be used when working with this population?
Identify a comprehensive plan to implement a preventive health project to meet the needs of the population. (4-5 pages). This should be an intervention which has been proven with evidence or is demonstrating effectiveness pending more rigorous evaluation. Review the literature to identify a model of best practice or promising practice for your intervention (access the librarian if necessary). Analyze the effectiveness based on the program’s outcomes. Apply to your community.
Identify at least two to three measurable objectives for your project-(refer below for SMART format)
Include community level indicators for your population (e.g. number of referrals for mental health, alcohol related ER visits, individuals on waiting list for in-patient admission, juvenile arrests, gunshot wounds, deaths due to automobile accidents, farm-related injury, deaths related to falls, delay to admission to hospice etc as your baseline to improve.)
Make the connection to a national Healthy People 2030 goals.
Discuss the program with a similar population you have identified and use as a model. Be sure to analyze and synthesize the characteristics of the model and the target population and cite sources.
Describe where your community intervention will take place

Current policies related to your project goals and entities responsible for policy making. Is this an individual, agency community county, city, state, federal? Who needs to know about this project to garner resources? On what level can you influence the policy?
Describe how you will evaluate your intervention. Remember to relate back to the objectives for the project. How will the objectives be measured? How was the intervention measured in your model program?
Describe recommendations for the future noting gaps or additional resources or programs to consider. (1page)
