Introduction: An introduction to the assignment is required at the start. • Provide a definition of the concept of assessment in mental health nursing and show your understanding of what and how we assess, including risk. LO4

• Discuss how the service user you have identified was engaged in the assessment. Consideration should be given to biological, psychological, and social aspects that might have might contributed to their mental health problems and presenting needs. LO1
• Examine how principles of recovery were used during the engagement and assessment of the service user. LO5
• Discuss how interpersonal skills were used to help provide person-centred care for the service user, supported by the wide body of literature on communication in mental health nursing.

Planning care for the service user:
• Define and show your understanding of the concept of care planning. Focus on how mental health nurses work with people in a way that values, respects and explores the ways that service users experience their mental health condition. LO2
• Discuss how care was planned for the service user you have identified and consider how this was linked to the principles of recovery. Describe and analyse how the collaborative care plan that was devised, was able to support and address their needs. LO2
• Identify and evaluate evidence-based interventions and provide rationale(s) for the choice of specific interventions. Refer to relevant policy guidance and frameworks to demonstrate an awareness of these interventions and their efficacy, to link theory and practice. LO6
• Consider the extent of service user’s/carer’s/other professionals (multidisciplinary, multi-agency) involvement in selecting interventions. LO5
• The completed care plan which addresses the two identified needs MUST be included as an appendix (Suggested: 1300 words)
Conclusion: Provide a summary of what you have discussed in the assignment (Suggested: 200 words).

Appendices: The appendices should include the care plans. Each appendix should include a care plan for a different need/ goal. Use a consistent format for the care plans and present them in the order in which they are discussed in the text of the assignment.

Based on Wrycraft (2015) which states that assessment can be done with the service user during the first few days of their inpatient admission, Mr Andrew was fully assessed at the next ward round a day after his admission by the Multi-disciplinary team (MTD) which includes Consultant psychiatrist, doctor, psychologist, social worker, welfare, the mental health Nurses and the student Nurses.
2. In order to comply with the NMC (2018) that says that mental health professionals must seek service user’s consent before commencing assessment. The consultant introduced student Nurses to Mr Andrew and asked if they should been involved in carrying out the assessment. Mr Andrew accepted for student Nurses to been included in the assessment process with him.
3. In application of Wrycraft (2015) definition of care planning, following assessment with Mr Andrew, information were gathered, Mr Andrew’s needs were identified, the MDT deliberates on the needs that were identified and care plan was drawn out

Coffey et al., (2019) also points out that involving service users in care plan and reviewing their care can help personalised care focused on recovery, with the aim of developing goals specific to the individual and designed to maximise achievements and social integration. NICE guideline recommends that mental health practitioners such as psychiatrists, mental health nurses, psychologists, and social workers worked with people using mental health services to jointly develop care plans, ensure that they are given copies of their care plan (NICE, 2022). In accordance with NICE (2022), the MDT worked in partnership with Mr Andrew throughout the care plan process, and they ensured that he was full involved in his care plan and a copy of his care was given to him.
