Mary is new graduate nurse on orientation at the local hospital . Mary and her preceptor are completing an admission for a client with multiple chronic illness. Mary gets overwhelmed with the multiple medications the client is taking. Medication reconciliation is a new process for Mary. She seeks guidance from the preceptor. The preceptor provides some written information and reviews the process verbally. Describe each of the following steps that Mary must perform to complete this process.
